A requirement by the Province of Ontario - All members of a hockey association must review the documentation regarding Concussion Awareness. The OWHA Rowans Law Acknowledgement Form must be signed by every hockey member.
Know the signs and symptoms of Concussions in yourself, your young athlete and fellow team mates.
The attached form, OWHA ROWANS LAW ACKNOWLEDGEMENT FORM must be printed and signed. Completed forms can be handed to your Team Manager/Coach, whom in turn will turn it over to the board Registrar Scanned forms may be emailed to registrar@oshawagirlshockey.ca
